Making a claim on your health plan is straightforward. First you should download our Cash Plan claim form or The One Scheme cash plan claim form:

The One Scheme Claim Form [pdf, 39Kb]
Please use this claim form if you are resident of the Republic of Ireland only and belong to HSF's The One Scheme.

Or if you would prefer to receive one by post, call the HSF health plan claims department on 1890 473 473 and they'll be happy to send you one.

Once you have completed the claim form, send it to us and we do the rest. If your claim is for professional fees we also need to receive the original receipts for any treatment or optical appliances you have paid for. Once you have done this, you should send the form and receipts back to the appropriate address shown at the top of the claim form.

We aim to have all claims settled promptly, particularly where you have had to pay for professional fees.
